Respected Sir,
With heartful sorrow, I want to mention you that my father who was a savings account holder with account no xxxxxxxx6765 in your bank has died in a car accident on xx/xx/2022. For your reference , I am attaching his death certificate along with this application form and other necessary documents.
As his sole nominee I request you kindly close his account and settle the amount of this account to my name. I have filled the nominee claim form and attached along with this letter. As witnesses I signed my aunt and uncle who are account holder of this branch. Their account numbers are aunt (xxxxxxxx3654) and uncle (xxxxxxxx9867). I also attached copy of the ID proof of the witnesses.
Also I attached an affidavit that certifying that I am one and only son of my deceased father. Copies of my ID and address proof are also attached along with this application for your kind consideration.
